Every quality professional can recite what changes in ISO 9001:2026. Very few can explain how it got there: who voted, on what, against which thresholds, and why a standard that members voted to keep unchanged in 2020 is being replaced six years later. That gap is not academic. Without the mechanism, you cannot tell a requirement from a rumour, you cannot judge whether a criticism you read online is serious, and you will plan around dates nobody has formally set. Here is the machine, with the current ISO 9001 revision as the worked example.

The mechanism in four figures

  • 5 years: the interval of the systematic review, counted from publication or from the last confirmation. It is what reopens the question.
  • 2 ballots actually decide: the enquiry stage (40) and the approval stage (50). The other stages prepare or execute.
  • 2 thirds and 1 quarter: a draft is approved if a two-thirds majority of the votes cast by P-members is in favour, abstentions excluded, and not more than one quarter of the total number of votes cast is negative (ISO/IEC Directives, Part 1).
  • 1 country, 1 vote: national standards bodies cast the votes, not the experts who draft the text.

A standard is voted, not decreed

Every International Standard goes through a systematic review five years after publication or after its last confirmation. National standards bodies then vote on three outcomes: confirm the text, revise or amend it, or withdraw it.

ISO 9001 went through that review in 2020, and the outcome was as close as it gets: 36 votes to confirm against 32 to revise. The Chartered Quality Institute described the agreement as reached by the narrowest of margins. Alongside it, a public user survey had collected 8,397 responses from 149 countries, and pointed the same way: a majority of respondents were not asking for a revision.

At that point the standard was confirmed. What followed is the part almost nobody recounts accurately, and it explains everything.

In 2021, subcommittee ISO/TC 176/SC 2 put a twin recommendation to the vote: confirm ISO 9001:2015 as it stood, and open a preliminary project to examine whether there was evidence for an early revision. Both parts were approved. A task group then drafted a design specification, and on the strength of that document SC 2 put the question to its members again, from 28 June to 28 July 2023.

This time the answer reversed. Result published on 3 August 2023: 36 votes to start a revision, 25 to affirm the existing standard, 17 abstentions. A dedicated working group, WG 29, first met in London in December 2023.

The honest reading is therefore not that members voted no and were overruled. The decision to confirm is precisely what commissioned the study, and opinion shifted between the two ballots. The fair criticism is narrower, and it stands: the question was put again, and the 2023 majority was reached among those voting, with seventeen members abstaining.

The lesson about the mechanism: a revision is not inevitable. It is voted on, more than once, and it can fail.

Six stages, and two ballots that can kill a project

Once a revision is launched, the text travels through a numbered sequence set by the ISO/IEC Directives, Part 1. ISO presents the development process as six stages numbered 10 to 60. The harmonized codes add a preliminary stage 00, which sits before stage 10, and two stages after publication: 90 for review, 95 for withdrawal.

Of those stages, only two decide: 40, the enquiry stage (DIS), and 50, the approval stage (FDIS). The others prepare the text or carry out a decision already taken.

The rule governing those two ballots is worth reading slowly, because most summaries of it in circulation are wrong. A draft is approved if a two-thirds majority of the votes cast by the P-members is in favour, and not more than one quarter of the total number of votes cast is negative. Two tests, with two different denominators: the first counts only P-members, the second counts every vote cast. Abstentions count in neither.

A negative vote with no technical reasons attached is not a vote. It is an opinion.

That single rule explains more about how standards get written than any amount of commentary: to carry weight you have to argue, and the argument has to be technical.

One last point of procedure, and ISO 9001 is a live illustration of it. Stage 50 is skipped automatically when the DIS has been approved and no technical change is introduced afterwards. It becomes mandatory again as soon as the draft incorporates technical changes arising from enquiry-stage comments, even if that DIS was approved. ISO 9001 did not skip stage 50: its record shows 50.00, 50.20 and 50.60. The text therefore did change after the enquiry stage, which is precisely what the comment process is for.

Reading a stage code

This is the most directly useful part. Every ISO project publishes its stage history, and the code answers one question only, but a decisive one: can the text still change. That is what lets you date any claim you meet online.

The full codification is published by ISO (international harmonized stage codes), and each project's history appears on its catalogue entry. Applied to the case at hand: ISO 9001 now sits at stage 60.00, publication under way. Anything still being argued today concerns a text that has been closed for months.

One distinction avoids the most common error here. The 2024 climate change amendment Amd 1:2024 is not a revision: it added one sentence to clause 4.1 and one note to clause 4.2. An amendment modifies the text in place and triggers nothing. What is coming is a revision, a new edition that cancels and replaces the previous one, and only that opens a transition period.

Experts draft, countries vote

This is the most frequent confusion on the subject. A working group writes the text, but the ballots are cast by national standards bodies, one vote per country. Each of those bodies forms its position in a national mirror committee, where manufacturers, service companies, certification bodies, consultants, public authorities and users argue before the country casts its single vote.

The consequence is direct: if your national position did not reflect your reality on the ground, that was decided at home, not in Geneva. And mirror committees are open. Complaining after publication costs the same effort as influencing at the enquiry stage, and produces nothing.

A word of caution on the numbers you will see quoted. A widely shared article reporting around three thousand international comments and a share of voting countries in favour actually describes the DIS of 2014, the one that led to ISO 9001:2015; it carries a recent last-updated date, which is why it keeps being cited as if it described the current cycle. Quoting a ballot more than ten years old as if it were this one is exactly the kind of error that gets corrected in public.

What the mechanism changes for you

Read the stage code before the commentary. A text at stage 40 is open and your comments count. A text at 60.00 is closed.

Check the date on every figure. Several of the most widely quoted numbers about this revision belong to the previous cycle.

Find your national mirror committee. It is the only point in the process where you can carry weight, and almost nobody uses it.

And do not expect ISO to give you your transition deadline. This follows directly from everything above: ISO publishes the standard, and says nothing about how long a certificate issued against the previous edition remains valid. That question is settled on the accreditation side. It used to belong to the International Accreditation Forum, which fixed the 2015 transition by resolution as early as 2013. But the IAF ceased to exist on 1 January 2026, merged with ILAC into Global Accreditation Cooperation Incorporated (Global ACI), and no binding Global ACI document on the transition has been published to date. National bodies are relaying a deadline of 30 April 2029 for ISO 14001:2026, but Accredia presents it as a proposed date, pending the final document, and UKAS attaches a written caveat to it. As a benchmark, IAF PR 7, the reference procedure on transitions, provides that where a transition period is not otherwise defined it is two years. Nobody expects that strict default to be applied. But an expectation is not a rule: build the plan on the shorter assumption and treat the rest as slack.
